All new patients/clients are given an assessment on their first treatment. The new patient intake form can be found on this page so that they can be printed off, filled out , and ready to take with for your first appointment. Alternatively we will provide forms at the clinic for you to fill out - this takes about 15 minutes, so please arrive a little early.

Tips for New Patients

In our office we see patients with for a wide variety of reasons from athletic performance conditioning for improving overall health and for a number of health conditions, and injuries for active rehabilitation. To help you get the most from your investment of time and money, here are some suggestions we would like to share with you:

1. Keep your appointments. Each visit builds on the ones before. If you are coming in for stretch therapy or injury rehabilitation or strength and conditioning, success in your program comes from consistency. Our recommendations to you are based on our years of experience and success with patients reaching their goals from performance enhancement to rehabilitation.

2. Do your homework. What you do at home and at work and in the gym on your own has a profound effect on your progress. Do your prescribed stretches everyday and exercises we give you on the scheduled days. Remember also to get your rest 8 hour of sleep helps your body recover faster. Proper nutrition is of utmost importance if you are struggling with the nutrition then ask us about the peak nutrition program.

3. Give it time. Healing or building strength, or losing weight is a process that takes time and is influenced by many factors. It takes time to retrain the muscles and soft tissues needed to support your body properly. Please be patient, the results will happen.
